MANDATORY PARTICIPATION EVALUATION & SPORTS CLEARANCE

In order to participate in athletic activities, students must first submit the Participation Evaluation AND Sports Clearance forms which are to be completed by a physician. Forms can be downloaded from VIDE.VI Division of Sports & Athletic (or just click on the links above). This process must be completed every year and can be done when students update their immunization cards.

IT'S MORE THAN JUST ATHLETICS

Participation in sports provides opportunities for improvement in more than just athletic abilities. Our athletes must also employ a variety of social and problem-solving skills in order to be successful as productive team members despite their position on any team.
