- Fill in their name and the student’s name in the opt-out statement
- Submit the signed form to the school
⏳ Deadline
If the school does not receive the opt-out form within two weeks, it is assumed that permission is granted for media release.

To resolve a school-related issue, follow these steps:

Start with the Teacher
Schedule a meeting to discuss your concern directly. -
Meet with the Principal
If the issue remains, request a written meeting. Allow 3–5 business days for a response. -
Reach Out to the Superintendent
Submit a letter summarizing the incident, your requested action, and include a copy of your letter to the principal. Allow 3–5 business days for a response. -
Contact the Commissioner of Education
If unresolved, submit a letter with all prior correspondence and clearly state your request. Allow 3–5 business days for a response. -
Request a Board Hearing
If needed, request a typed, notarized hearing within 30 business days of the Commissioner's response. Gather supporting documents and witnesses to strengthen your case.
