School Clinic/Nurse

At the start of the school year, each student should submit an updated immunization record to the school nurse. Students who intend to participate in athletics should also submit the  Participation Evaluation AND Sports Clearance forms which are to be completed by a physician. Forms can be downloaded from VIDE.VI Division of Sports & Athletic (or just click on the links above). This process must be completed every year and can be done when students update their immunization cards.
